Pay Your Account Online
For your convenience we now offer you the ability
to pay your account online using PayPal. If you need to know your
current account balance, please refer to your most recent billing
statement or contact our office for personal assistance. If you

Finance Options
Our first priority is to provide you with the
highest quality dental care possible. The rising cost of dental
care has recently been a subject of great interest to us all. We
too, are interested in minimizing costs so that you and your family
can receive quality, individualized care for a reasonable fee. With
the goal of keeping costs affordable we ask our patients to be prepared
with payment at each visit.
The following explains our payment options.

A 5% discount deducted from
your fee, for services over $250.00, if you pay in full at the
time of initial treatment (first visit) by check, cash or money
order. If you have insurance we will gladly arrange for your
insurance company to reimburse you directly.

Pay As You Go: Payment in full on the day
services are rendered made by MasterCard, Visa, Discover, American
Express. This option will let you know how much your monthly
payments will be so that you can budget your payments comfortably.

| C. |
Pay As You Go: Payment in full on the day
services are rendered by personal or business check. We ask
that you provide us with your driverŐs license number for our
banking purposes.

| D. |
Insurance on Assignment: We will gladly
file your insurance claim as a service to you and will do
our best to legally maximize your benefits. Any portion estimated
not to be covered by your insurance company is due in full
at the time of service.
*Some insurance companies will only
send payment directly to the patient. If this is the policy
of your insurance company then you will be responsible for
payment in full at the time of service. Your insurance company
will reimburse you directly.
